Insurance provides a specific number of benefits designed to cover funeral expenses when the insured dies. It has guaranteed emissions, which means your team doesn't need a medical exam or health questionnaire to get it.
It is designed for groups of 10 or more employees, and 25 members of a union or association. In this case, the couple and their dependent children may be eligible, and the group should choose the type of plan that covers them.

- Specific number of benefits for the payment of funeral expenses.
Contract dispositions
- The person who incurred the expenses will be paid upon receiving the original invoice or a copy certified by the Funeral Home that presents the funeral expenses or burial expenses of the insured.
- COSVI may disburse payment for funeral expenses to the Funeral Home if the Policy Claimant authorizes it.
- Group of 10 employees or more and 25 or more members of the union or association.
- The Partner and Dependent Children may be eligible for compensation, in this case, the group must select the type of plan to cover them.

How can I establish my claim?
To submit a claim you must complete the Claim Application and accompany it with the necessary documents according to the insurance policy you have purchased. For more information you can access the Claims section. For your convenience, you can advance these documents by e-mail service.reclamaciones@cosvi.com, via fax to 787-200-2574 or deliver them to any of our offices. An important fact for SINOT cases is that SINOT claim applications require original documents.
What should I do if I want to reinstate my life policy?
You just have to fill out the Reinstatement Application and send it in original format, with the payment of all premiums owed by regular mail to: Puerto Rico Life Insurance Cooperative, COSVI Attention Issuance Department PO Box 363428 San Juan, P.R. 00936-3428. You can also deliver it to any of our offices or through your Authorized Insurance Representative.
What if I want to purchase an additional cover?
You can contact the Life Insurance Cooperative of Puerto Rico, COSVI at 787-751-7373 where an Authorized Representative will guide you on the insurance policies and endorsements that we have available. You can also contact your insurance agent directly.
How can I establish an automated pay on my account?
To set up an automatic payment, you should complete a card bank form, which should be accompanied by a voided check or proof from a financial institution.
If I no longer belong to the group with which I acquire my policy, can I continue with my cancer coverage?
No, you should apply for an individual or family cover.
