Available plan: Groups Will pay the monthly payment of the loan because of individual dismissal or for massive employment, general strike, termination of the job by the company or employment dispute, or company is closing temporarily or permanently.
Benefits
The original term of the loan/ Benefits paid
- 1-12 months/ 4 payments
- 13-25 months/ 6 payments
- 26-36 months/ 8 payments
- 37-48months/ 12 payments

Contract dispositions
- Must work 30 hours per week for 12 consecutive months before the effective date.
- Actively worked as a paid employee at the same company for the first 90 days of employment.

What should I do if I want to reinstate my life policy?
You just have to fill out the Reinstatement Application and send it in original format, with the payment of all premiums owed by regular mail to: Puerto Rico Life Insurance Cooperative, COSVI Attention Issuance Department PO Box 363428 San Juan, P.R. 00936-3428. You can also deliver it to any of our offices or through your Authorized Insurance Representative.
Are the nuclear tests covered under the School and the Special Accidents Policy?
No, only covers X-Ray.
Can a person buy a cancer and pernicious illness policy a person that has had a cancer diagnosis?
Yes, they are eligible if they had not received any treatments in the past 10 years, and it doesn’t happen within the first 30 days of the policy and the date of effectiveness.
Does COSVI Cancer policy include prevention exams?
Yes, we can reimburse preventive exam expenses if the policy is active for two months.
What should I do if receive information that my policy is late?
Communicate with your Authorized Representative or Customer Service Specialist to make the payment. Remember, to have your policy member information on your hand to facilitate the identification.
