Individual Life Insurance and IRA/Annuities

To proceed with your claim, we will need you to provide some important documents. Please make sure to include the following documents:

In the case of death:

• Benefits application completed and signed by the claimant.
• Birth certificate
• Death certificate (Model RD-77)
• Police report
• Autopsy report

In the case of disability:

• Request for benefits completed and signed by the insured
• Birth certificate
• Medical certificate
• Medical evidence
